(here in after “Association”), to the Association for its purchase in accordance with the By-laws of the Association. I understand that the “stated value” of $300.00 less money owed the Association will be paid to me upon receipt of this form, unless I am converting my certificate to a Life Membership, whereby the remuneration will be retained by the Association.
